Please take into consideration that when outlining your child’s extra needs, requirements and differences you must do so in detail. The person reading your form needs to fully understand what it is like for you to be caring for a person with a hand difference. Your family doctor, paediatrician or clinic doctor/surgeon may fill in the Treating Doctor’s Report. Also, your occupational therapist may offer further assistance in completing the rest of the form.
